The Birth to Three Service Coordinator will coordinate the delivery of Birth to Three services across agency lines and serve as the single point of contact in helping a family obtain the services the child and family need. These services are provided in collaboration with families, care providers, and other professional staff and reflect developmentally appropriate, relationship-based intervention in natural environments (home or community-based) through the implementation of evidence-based practices. 

 

JOB-SPECIFIC REQUIRED COMPETENCIES:

• Complete a family assessment to determine strengths, resources, needs and concerns.
• Serve as the primary contact for families enrolled in Birth to Three.
• Coordinate the enrollment process for each child, including completing necessary forms.
• Act as a guide for the Eligibility/IFSP team process with Birth to Three staff,  parents, and related community partners.
• Assist in the determination of program eligibility based upon criteria outlined in HS-90.
• Regularly reconvene the IFSP team.  Review the IFSP with the child’s parents and Birth to Three staff involved make changes at least annually.
• Facilitate all subsequent IFSP team meetings.
• Provide ongoing support to families enrolled in Birth to Three with a minimum contact via telephone monthly and face-to-face contact quarterly.
• Assist and enable families to receive and understand Birth to Three procedural safeguards under HS-90.
• Demonstrate an understanding of the development of young children and how children with varying abilities learn.
• Demonstrate an ability to create learning experiences in everyday family routines.
• Facilitate the development of a transition plan and assist families with transition to school-age/community services as the child nears age three.
• Collaborate and work effectively as a team member with Birth to Three therapists to support the intellectual, social, motor and communicative needs of the child, as well as the personal needs of his/her family.
• Demonstrate an understanding of Birth to Three “Best Practices,” and work diligently to achieve quality indicators set forth by OSEP and the State Birth to Three office. 
• Complete case notes, monthly reporting and PPS documentation in compliance with Federal, State, and Medicaid regulations.

TRAINING, LICENSURE AND EXPERIENCE:

• Graduation from an accredited four-year college or university with a BS or BA in a Human Services-related field is preferred.
• Prior experience working in a home-based setting with young children, young children with disabilities, and families, and/or case management is preferred.
• Valid State of WI Driver’s License.

PHYSICAL ABILITIES REQUIRED TO PERFORM ESSENTIAL JOB DUTIES:

• Staff will be required to travel to homes or locations throughout the county,
• Services may be provided in unsanitary conditions, 
• Staff may encounter second-hand smoke, and/or
• Family pets may interfere with personal allergies (as applicable).
